Flu Vaccines at School

Parents/Guardians,

The Sevier County Health Department will be offering flu vaccines for students at school this fall. The date has not been determined yet, and we will share the clinic date as soon as it is scheduled.
If you would like your student to receive the flu vaccine at school, you must complete the online consent form using the QR code or RedCap link provided in the attached information.

Please note: A separate consent form must be completed for each student receiving the vaccine.
